The Heinz Simonitsch School operates from locations: 1225 Ironshore Drive, Montego Bay, and Half Moon Shopping Village, Rose Hall. 1. Choose the admission form for the child's program: Early Years Programme (EYP), Junior School, Senior School, or Sixth Form. The Heinz Simonitsch School provides separate applications for each program, and the Junior School uses a Registration Form to begin the admissions process. 2. Complete all sections of the Registration Form and write 'Not Applicable' or 'N/A' in any section that does not apply. The form requests the child's full name, age, date of birth, and gender (Male or Female), and asks for personal information such as allergies, likes/dislikes, eating, sleeping habits, and any other information. 3. Provide Immunization Information and attach a copy of the immunization record. The form asks whether the child has had ALL shots and lists DPT, POLIO, MEASLES, TUBERLIN TEST, and RUBELLA (German Measles). 4. Provide Family Information for both parents/guardians, including Mother's Name, Mother's Address, Email Address, Telephone Numbers, Occupation, Work Address, and Father's Name, Address, Email Address, Telephone Numbers, Occupation, and Work Address. 5. In Case of Emergency Notify: supply the emergency contact's name and address, and a telephone number; provide the name and contact number of the child's doctor; indicate whether the Centre's Paediatrician may be called if necessary; confirm whether the child is free of contagious disease and whether they suffer from illnesses such as Asthma, Epilepsy, or Hay Fever; name persons authorized to collect the child from school. 6. Academic Program Entering: indicate the grade level registering for and provide Academic Goals; the form requires the Parent's Signature and Date. 7. For Internal Use Only: the section includes spaces for the School's Decision, the School's Official, and the Date. 8.
