To enroll a child, parents/guardians must submit the Application for enrolling a child to the Public Organization International Ukrainian School of Lithuania, which covers Vilnius, Kaunas, Klaipėda, Šiauliai, Panevėžys and places the child in Lithuania's register of students for grades 1–11. An Application for determining the educational level is submitted if the student lacks documents for more than the two previous academic years. 2. An agreement on the provision of educational services, and an agreement on the processing of personal data, are signed for the academic year. 3. Together with the contract, parents submit the following documents: a copy of the child's birth certificate; a copy of the child's residence permit; a copy of the residence permit of one of the parents/guardians; a copy of the passport of one of the parents/guardians and the child (if any); a document on previous education (report card, certificate of achievement, etc.); for grades 10–11, a copy of the Certificate of Basic Secondary Education; a medical certificate; and the original personal file and original Certificate of Basic Secondary Education (for grades 10–11). 4. After signing the contract, an invoice for the payment of 50 euros is issued. 5. At the request of parents, certificates approved by the administration are provided in Ukrainian and Lithuanian with confirmation that the child is studying at the International Ukrainian School of Lithuania.
