Introduction

1. To enroll a child, parents/guardians must submit two applications: an application for enrolling the child in the Public Organization 'International Ukrainian School of Lithuania' and, if the student lacks documents for more than two previous academic years, an application for determining the child's educational level. The school operates in Vilnius, Kaunas, Klaipėda, Šiauliai, and Panevėžys, and the child is entered in the Lithuanian student register. The Šiauliai campus is located at Architektų g. 1, Šiauliai. Enrollment is for students in grades 1-11. 2. An agreement on the provision of educational services and an agreement on the processing of personal data are signed for the academic year. 3. Parents provide the following documents with the contract: the child's birth certificate; the child's residence permit; the residence permit of one of the parents (representatives); the passport of one of the parents (and the child, if any); documents on previous education (report card, certificate of achievement, etc.); for grades 10-11, a copy of the Certificate of Basic Secondary Education; a medical certificate; the original personal file and the original Certificate of Basic Secondary Education (for grades 10-11). 4. After signing the contract, an invoice for the payment of 50 euros is issued. 5. At the request of parents, certificates approved by the administration are provided in Ukrainian and Lithuanian with confirmation that the child studies at the International Ukrainian School of Lithuania (MUSH).
