It includes sections for Extra Curricular Activities and a Declaration, and requires the parent/guardian's contact information. Step 2. Attach supporting documents: The form requires attachments of the child's Birth Certificate, Previous School Leaving Certificate, Result Slip, and two Passport-sized photos.
