This information is used to verify the payment and link it to your application. The admission forms are available online, at the School premises, and at examination centres across Nigeria. 2. Upon confirmation of payment, you will be provided with a username and password to access the online admission form, or you may visit the School to pick up the admission form. 3. The admission forms can be obtained online, at the School premises, and at examination centres nationwide, and the process continues with the entrance exam. 4. Examination centres include Premiere Academy FHA Lugbe Abuja; Kids N Us School, Calabar; School of Infant Jesus and Mary, Enugu; Sacred Heart School, Kaduna; Premiere Eye Clinic, Victoria Island Lagos; Westhood Park Nursery and Primary School, Jos; Montessori International School, Port Harcourt; Sanvee International School, Uyo; Mount Olive School, Onitsha; Ideal Nur/Pry School, Eket; St. Rita Nursery & Primary School, Makurdi; The Cradle School, Warri.
